Key Responsibilities\n

As a Licensed Professional Counselor at Invasive Care Consultants, your key responsibilities will include:

• Conducting thorough and ongoing patient assessments to identify individual needs effectively.

• Facilitating communication among supervisors, direct care staff, families, and other stakeholders regarding patient progress and treatment plans.

• Preparing and presenting a tentative schedule of appointments, home visits, and observations to monitor patient care delivery.
\n
• Coordinating home visits for comprehensive assessments, including chart reviews, staff interviews, and data analysis.
\n
• Implementing and monitoring the effectiveness of personalized treatment plans to ensure they meet patient needs.
\n
• Participating actively in Interdisciplinary Team Meetings and relevant discussions to enhance patient support.
\n
• Documenting progress through detailed monthly and quarterly notes that comply with regulatory standards.
\n
• Providing ongoing training to staff and team members to ensure they are equipped to implement treatment plans effectively.

• Maintaining accurate and organized records to ensure compliance and facilitate information sharing with relevant parties.

• Acting as a liaison with public and mental health entities to advocate for patients and contribute to program development.

Compensation and Benefits\n

The salary for this position ranges from $90,000 to $100,000 per year, commensurate with experience and caseload availability.
