Key Responsibilities

• Evaluate and treat patients recovering from injury, disease, or surgery in their own homes, employing a variety of therapeutic techniques.
• Develop individualized treatment plans that incorporate exercises, hands-on therapy, stretching maneuvers, and the use of specialized equipment aimed at alleviating pain and improving mobility.
• Document patient treatments, responses, and progress meticulously to ensure continuity of care and effective outcome tracking.
• Regularly reassess and modify treatment plans based on patient responses and evolving therapeutic goals, enhancing the recovery journey.
• Provide education and training to patients, families, and caregivers on the use of assistive devices and proper physical therapy techniques.
• Engage in professional development activities to maintain up-to-date knowledge in the field by attending workshops and participating in professional societies.

Patient Population / Client Focus

The ideal candidate will be experienced in working with diverse patient demographics, including children and adults facing multifaceted health challenges. Our patients range from those requiring rehabilitation after orthopedic surgery to individuals recovering from neurological conditions, each requiring distinct therapeutic approaches to achieve their personal health goals.

Qualifications

• A Bachelor's Degree in Physical Therapy; a Doctorate in Physical Therapy (DPT) is preferred for this position.
• A valid State-issued license to practice Physical Therapy.
• Exceptional communication skills, both written and verbal, to effectively interact with patients and their families.
• Technical proficiency in electronic medical record systems to maintain accurate documentation.
• Highly adaptable and flexible in the face of varying patient needs and schedules.
• Must possess a commitment to continued professional development and learning.

Preferred Experience

While a minimum of two years of relevant experience is preferred, new graduates with a strong foundation in clinical practice may also be considered. Prior experience in home care or community-based therapy settings is a plus but is not mandatory.
