Key Responsibilities\n

• Engage in collaborative and ongoing communication with physicians and other healthcare professionals to provide comprehensive updates on patient progress, needs, and concerns.

• Maintain accurate, timely clinical records and progress notes as per departmental policies, utilizing our electronic health record (EHR) systems effectively.

• Develop individualized treatment plans, including detailed discharge plans in collaboration with multidisciplinary team members, based on thorough psychosocial assessments.

• Provide direct counseling services and crisis intervention to clients facing emotional and behavioral challenges, tailoring therapy approaches to each individual’s needs.

• Assist clients with discharge planning and care coordination, ensuring they have access to necessary community resources and services post-treatment.

• Engage with clients' family members and support systems to facilitate a supportive network during the treatment and discharge planning process.

Patient Population / Client Focus\n

You will work with a diverse client population, including individuals grappling with a range of mental health issues such as anxiety, depression, trauma, and substance use disorders. Qualifications\n

Compensation and Benefits\n
