Work Environment\n

This is a remote position, allowing you to connect with clients and families from the comfort of your home while maintaining a flexible schedule. The full-time hours are typically set from Monday to Friday, between 8:00 AM and 4:30 PM, with occasional evening or weekend hours required based on client needs. Key Responsibilities\n

• Conduct in-depth psychosocial assessments to evaluate clients' emotional, mental, and social needs, ensuring a comprehensive understanding of each client’s situation.

• Develop personalized care plans in partnership with clients and their families, aligning with their specific goals, preferences, and cultural values.

• Provide compassionate counseling and emotional support, helping clients and their families navigate significant life transitions, including illness, grief, and loss.

• Advocate for clients by ensuring their rights are respected, and they have access to necessary community resources and services.

• Conduct regular check-ins with clients to monitor their well-being, assess living conditions, and address safety or health concerns.

• Facilitate effective communication among family members involved in care to promote cooperation and address any conflicts that may arise.

• Educate clients and families about community resources, support groups, and engagement programs to enhance social interaction and overall quality of life.

• Maintain detailed and accurate documentation in electronic health record (EHR) systems, including assessments, care plans, and progress updates.

In this role, you will focus primarily on seniors receiving hospice care, who are often facing complex emotional and social challenges. Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• A minimum of two years of direct social work experience, particularly in a hospice, geriatric, or community-based setting.

Compensation and Benefits\n
