Key Responsibilities\n

• Conduct comprehensive assessments of patients' psychosocial needs to develop tailored treatment plans that incorporate short-term counseling, referrals, and crisis intervention strategies.

• Assist clients in navigating administrative processes by guiding them through applications for entitlements, supplemental insurance, and other benefits aimed at reducing reliance on emergency assistance programs.
\n
• Facilitate access to pharmaceutical assistance programs, ensuring patients can afford their medications through partnerships with community organizations and pharmaceutical companies.
\n
• Act as a patient advocate when communicating with external agencies to ensure clients receive necessary resources and support.
\n
• Maintain diligent and accurate patient records, including preparation of reports and documentation to meet compliance standards and support clinical evaluations.
\n
• Coordinate transportation assistance for patients needing access to in-person appointments or services, thereby removing barriers to care.
\n
• Participate actively in clinical team meetings to discuss case progress, treatment strategies, and share best practices with colleagues.

Patient Population / Client Focus\n

This position primarily serves adults experiencing a variety of mental health challenges including, but not limited to, anxiety, depression, trauma, and substance use disorders. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of an active or independent license such as Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• At least two (2) years of professional mental health counseling experience, demonstrating proficiency in various therapeutic modalities and a strong understanding of clinical processes.

• Experience in crisis intervention and management, with a proven ability to handle high-stress situations effectively.

• Familiarity with community resources and the ability to strategically network with local agencies to support patient needs.

Compensation and Benefits\n
