Key Responsibilities\n

• Crisis Management: Conduct thorough crisis assessments and maintain response protocols to support participants during critical times, ensuring timely coordination with county crisis services.

• Care Coordination: Manage transitions between hospital/inpatient services and our programs by collaborating with healthcare staff to ensure smooth discharges and program transfers.

• Risk Assessment & Documentation: Complete detailed risk assessments and safety plans for individuals at heightened risk of crises, ensuring documentation is finalized the same day to maintain compliance and quality.

• Incident Reporting: Submit critical incident reports as required, adhering to timelines to facilitate transparency and accountability in our services.

• After-Hours Support: Participate in an alternating after-hours phone system to provide ongoing care coordination support, enhancing access to services for participants.

• Proactive Engagement: Work hands-on with participants to develop strategies that minimize the need for crisis interventions while fostering resilience and self-sufficiency.

• Additional Tasks: Perform other relevant duties as assigned to support the smooth operation of our mental health services.

Qualifications\n

• A Master's or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited program (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).
