Key Responsibilities\n

• Conduct thorough assessments and evaluations of clients to develop tailored treatment plans that align with their personal goals.

• Utilize digital tools to maintain accurate and detailed records of sessions and treatment plans, ensuring that all data is accessible to the care team.

• Facilitate follow-up appointments and in-home evaluations when required, ensuring that all aspects of patient needs are continuously addressed.

• Prepare and present monthly and quarterly progress reports, showcasing patient outcomes and program effectiveness.

• Lead training sessions for colleagues on new therapeutic techniques, documentation best practices, and program initiatives to foster team development.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.
