This position is set in a home care environment, providing you with the opportunity to work closely with patients in a comfortable and familiar setting. You will be a vital part of our committed team that prioritizes patient-centric care, working from Monday to Friday with a structured 40-hour work schedule. Key Responsibilities

• Conduct comprehensive evaluations to determine patients' physical therapy needs and formulate individualized treatment plans based on physician referrals.
• Implement tailored therapeutic interventions to address a range of issues, utilizing modalities that align with patients' therapeutic goals.
• Maintain effective communication with patients and caregivers, providing education about conditions, treatment options, and home exercises.
• Demonstrate exemplary customer service to enhance patient satisfaction and contribute positively to the clinic's reputation.
• Collaborate with an interdisciplinary team to maximize patient outcomes by integrating evidence-based practices into treatment protocols.
• Document patient progress meticulously, ensuring that all clinical notes are up-to-date and reflect adherence to the physician's plan of care.
• Meet or exceed established utilization goals as well as specific business performance objectives.

Patient Population / Client Focus

Our patients come from diverse backgrounds and require a variety of therapy specialties, including but not limited to orthopedic rehabilitation, pediatric therapy, neurological recovery, and sports medicine. In this role, you will actively engage with individuals at various stages of life, adapting your therapeutic approaches to fit their unique circumstances. Your contributions will significantly impact their functional abilities, promoting independence and quality of life through skilled care.

Qualifications

• Bachelor's degree in Physical Therapy from an APTA or CAPTE-approved program.
• Valid State License in Physical Therapy to practice in California.
• Basic Life Support (BLS) Certification as recognized by the American Heart Association.
• Excellent written and verbal communication skills, essential for effective patient and team interactions.
• Proficiency in word processing and familiarity with electronic medical records (EMR) systems.
• Demonstrated ability to build strong therapeutic relationships with patients, ensuring they feel supported and understood throughout their treatment.

Preferred Experience

Compensation and Benefits
