Work Environment

This full-time position requires you to work onsite as you deliver care directly in patients' homes. You will be expected to typically work 40 hours per week, including variable weekends, enabling you to provide flexible care tailored to each patient's individual needs. The home care environment allows for personalized interactions and enables you to assess and respond to each patient's unique home dynamics, promoting more effective rehabilitation.

Key Responsibilities

• Conduct comprehensive examinations and evaluations to develop individualized treatment plans tailored to each patient's specific functional limitations and disabilities.
• Deliver hands-on therapy and education, implementing treatment strategies that may include exercises, manual therapy, and patient education on injury management and prevention.
• Maintain accurate documentation of evaluations, treatment plans, and patient progress, ensuring compliance with all applicable laws and professional standards.
• Collaborate with interdisciplinary team members, including other healthcare providers and family members, to ensure a holistic approach to patient care.
• Participate in community outreach programs to elevate the profile of our services and promote awareness around rehabilitation and wellness.
• Engage in continuous professional development through ongoing training and education to refine skills and competencies.
• Supervise and mentor support staff to enhance productivity and quality of care.

Patient Population / Client Focus

Our physical therapist will work with a diverse population of patients ranging from post-operative orthopedic cases to those with chronic pain and mobility challenges. Each patient presents unique challenges, requiring you to exercise clinical judgment and empathy, focusing on individualized care that takes into account age, cultural background, and specific health needs.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• State licensure as a Physical Therapist is mandatory for this role.
• Basic Life Support (BLS) certification is required.
• Exceptional verbal and written communication skills to effectively interact with patients and healthcare professionals.
• Proficient in electronic documentation and word processing software, supporting efficient patient care and billing processes.

Preferred Experience

While a Bachelor's degree is required, candidates with a Doctorate in Physical Therapy (DPT) or additional certifications related to specialty areas (manual therapy, geriatrics, etc.) will be preferred. Previous experience in home care or similar settings will be highly regarded as it provides insights into delivering personalized patient interactions and managing varied home environments.
