Key Responsibilities

• Conduct comprehensive patient evaluations, including a thorough review of medical history and specific assessments, to develop personalized treatment plans aimed at recovery and rehabilitation.
• Collaborate with patients and their families to explain evaluation findings, establish treatment goals, and integrate their preferences into the care plan.
• Implement treatment protocols effectively, adjusting techniques based on individual patient progress and evidence-based practices in line with current healthcare standards.
• Facilitate interdisciplinary teamwork by consulting with additional healthcare providers when necessary and recognizing crucial changes in a patient's condition that may require adjustments in the treatment plan.
• Ensure accurate and timely documentation of patient interactions, including evaluations, treatment progress, discharge plans, and follow-up care procedures to maintain comprehensive patient care records.
• Exhibit strong time management skills to meet productivity expectations while maintaining a structured and accurate schedule of patient appointments and meetings.
• Contribute positively to department goals and objectives, remaining compliant with governmental and accreditation standards to elevate the quality of care provided.
• Engage in ongoing professional development by pursuing continuing education opportunities and mentoring students or new clinicians within the facility.
• Promote exceptional patient experiences through adaptable methods that ensure access to appropriate therapeutic care, thereby fostering a supportive clinical atmosphere.
• Participate in marketing initiatives aimed at enhancing community presence and educating the public about our outstanding services.

Patient Population / Client Focus

In this role, you will primarily work with a diverse group of patients dealing with orthopedic conditions, including sports injuries, post-operative rehabilitation, chronic pain, and age-related musculoskeletal disorders. At Active Life Physical Therapy Center, we focus on individualized treatment plans that are tailored to the unique needs of each patient, enhancing their chances for successful recovery.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program, equipping you with the necessary knowledge and skills for this position.
• State licensure as a Doctor of Physical Therapy (DPT) issued by the Physical Therapy Examining Committee.
• A minimum of two (2) years of hands-on experience in a clinical setting, demonstrating your expertise in patient care and rehabilitation methodologies.
• Current CPR certification to ensure patient and team safety during all interactions.

Preferred Experience

• Experience working in outpatient therapy settings, particularly focusing on orthopedic rehabilitation.
• Familiarity with electronic medical records (EMR) systems and documentation best practices in physical therapy.
• Proven ability to engage effectively with a diverse patient population and cultivate strong therapeutic relationships.
