Work Environment

Key Responsibilities

In your role as a Senior Home Care Physical Therapist, you will:

• Direct physical therapy treatments aimed at preventing disability, alleviating pain, and restoring muscle function, while adhering to personalized plans of care as prescribed by a physician.
• Evaluate and assess patient function through diagnostic tests, ensuring a comprehensive understanding of their capabilities to formulate effective treatment strategies.
• Supervise physical therapy assistants, ensuring that therapy practices align with established plans and best practices, while also maintaining compliance with state laws.
• Conduct thorough evaluations of home environments, recommending modifications to enhance patient independence and promote safety.
• Administer a variety of treatments, including manual therapeutic exercises, traction, and other modalities to facilitate optimal recovery.
• Prepare and maintain precise clinical documentation, including initial evaluations, care plans, progress notes, and discharge summaries, to support transparency and continuity in care.
• Collaborate with an interdisciplinary team during case conferences, actively participating in discussions to plan and enhance patient care and outcomes.

We serve a diverse patient population, focusing on individuals requiring physical rehabilitation in their homes. Patients may include those recovering from surgery, injury, or chronic conditions that impact their mobility or functional capabilities. Qualifications

The ideal candidate will hold a Doctorate in Physical Therapy (DPT) and meet the following criteria:

• Possess a valid and current physical therapy licensure in the state of Virginia, with no disciplinary actions.
• Current CPR certification is required to ensure patient safety.
• A minimum of one year of experience in physical therapy, preferably in a home care or acute care setting, showcasing your ability to work independently.
• Demonstrated commitment to clinical excellence, quality, and integrity, paired with excellent interpersonal skills to foster rapport with patients and their families.
• A strong aptitude for critical thinking, decision-making, and creative problem-solving, particularly in adapting care plans to meet unique patient needs.

Preferred Experience

While not mandatory, previous experience in home healthcare or outpatient physical therapy settings is highly desirable. Familiarity with community resources for patients and the ability to work collaboratively in a multidisciplinary team will be considered advantageous.
