Key Responsibilities

Your daily responsibilities will encompass a variety of tasks aimed at facilitating patient recovery:

• Conduct thorough Physical Therapy evaluations for patients, identifying specific needs and formulating effective treatment plans.
• Develop individualized therapy programs that align with patients' goals and capabilities, and execute these plans with precision.
• Regularly reassess patients' progress through detailed clinical evaluations, modifying treatment strategies based on observed progress and feedback.
• Complete comprehensive documentation, including SOAP notes and progress summaries, utilizing our electronic medical record system promptly to maintain accurate patient histories.
• Educate patients and their families about the recovery process, potential challenges, and strategies to support ongoing rehabilitation efforts.
• Meet established utilization goals, supporting the operational objectives of the clinic while enhancing patient outcomes.
• Collaborate effectively with interdisciplinary teams, sharing insights and supporting patients receiving multiple therapeutic services.

Patient Population / Client Focus

Qualifications

To be successful in this role, candidates must meet the following criteria:
