Rhythm & Motion Physical Therapy and Wellness is actively seeking a skilled and dedicated Physical Therapist (PT) for our rehabilitation facility located in Lincroft, New Jersey. Work Environment

Our rehabilitation facility in Lincroft has been designed to create a positive atmosphere where staff and patients can thrive. As a member of our team, you will work onsite alongside other dedicated professionals in a space filled with state-of-the-art equipment and a wealth of resources. Operating hours are Monday through Friday, from 8:00 am to 5:00 pm, allowing for a structured yet collaborative work schedule while ensuring high levels of patient accessibility and satisfaction.

Key Responsibilities

• Adhere to state practice acts and regulations governing the role of physical therapists, ensuring compliance in all clinical activities.
• Conduct thorough examinations and evaluations of patients to develop individualized treatment plans aimed at restoring function and enhancing mobility.
• Implement and adjust treatment protocols based on patient progress, clinical findings, and interdisciplinary feedback, ensuring a tailored approach to care.
• Maintain meticulous documentation of patient care activities, including initial evaluations, treatment notes, progress summaries, and discharge documentation to guarantee continuity of care.
• Provide educational support to patients and families regarding the rehabilitation process, discharge planning, and long-term health and wellness strategies.
• Collaborate with healthcare professionals within and outside the clinic to address complex patient needs, making timely modifications to care plans as necessary.
• Actively participate in facility meetings and contribute to quality improvement initiatives by providing insights and strategies to improve patient access and outcomes.

Patient Population / Client Focus

In this role, you will primarily serve patients with orthopedic injuries, post-surgical rehabilitation needs, and chronic pain conditions. Your expertise will help guide patients from initial assessment through recovery, ensuring they receive personalized and effective care to meet their unique rehabilitation goals. Our diverse clientele ranges from active adults to older individuals seeking to regain their independence and enhance their quality of life.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Active and valid state license as a Physical Therapist in New Jersey.
• Two years of outpatient orthopedic experience, demonstrating proficiency in managing a variety of patient cases.
• Current CPR certification to ensure patient safety during treatment.

Preferred Experience

• Experience with manual therapy techniques and modalities relevant to orthopedic rehabilitation.
• Familiarity with telehealth platforms and electronic health record systems for documentation and patient management.

Compensation and Benefits
