Key Responsibilities\n

• Conduct comprehensive assessments of clients to identify their unique mental health needs, ensuring tailored intervention plans are developed.

• Maintain clear and open lines of communication with supervisors, direct care team members, and clients’ families to discuss patient progress and necessary adjustments to treatment plans.
\n
• Prepare and share a tentative weekly schedule of appointments, home visits, and required observations to monitor patient progress effectively.
\n
• Coordinate and execute home visits that include observations, chart reviews, and staff interviews, allowing for a holistic view of the client's situation.
\n
• Ensure that implementation of treatment plans aligns with best practices and is delivered consistently across all patient interactions.
\n
• Participate actively in Interdisciplinary Team Meetings, contributing insights and recommendations to enhance patient care.
\n
• Document ongoing progress through monthly and quarterly reports, helping to track client development and outcomes.
\n
• Facilitate training sessions for staff to ensure they are well-versed in the necessary program implementation and documentation requirements.
\n
• Provide ongoing support through follow-up interviews and additional home visits as needed to ensure continuity of care.
\n
• Act as a liaison among the mental health community and the public, working collaboratively to formulate and implement development objectives.
\n
• Assist in the creation of training materials intended to improve care programs and policies for individuals served.

Patient Population / Client Focus\n

In this role, you will work with a diverse range of clients, including adults and youth, facing challenges such as anxiety, depression, trauma, and substance use issues. Your ability to engage with a variety of demographics and tailor your therapeutic approach based on individual circumstances will be critical to achieving positive outcomes.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• An active and unrestricted license as a Professional Counselor (LPC) in Colorado is mandatory.

• A minimum of two years of clinical counseling experience is essential, with a focus on direct patient care.

• Experience in family therapy, couples therapy, substance misuse counseling, or trauma-informed care is highly valued.

Compensation and Benefits\n
