Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, adolescents, and families.

• Utilize graduate-level clinical social work and counseling techniques tailored to meet the unique needs of each client.

• Develop and implement individualized treatment plans in conjunction with patients and families, ensuring adherence to clinical goals.

• Engage in regular communication with interdisciplinary teams through documentation, patient rounds, and case meetings to coordinate care effectively.

• Provide ongoing case management services, collaborating with both hospital and community professionals to facilitate comprehensive support.

• Document case notes and treatment progress ethically and punctually as per departmental standards.

• Incorporate evidence-based practice, including Cognitive Behavioral Therapy (CBT) modalities, into treatment plans to enhance patient outcomes.

Qualifications\n
