Key Responsibilities\n

• Conduct initial assessments with clients to understand their personal goals, preferences, and available treatment options, preparing the groundwork for effective rehabilitation plans.

• Develop customized rehabilitation plans that take into consideration each client’s educational background, personal aspirations, and physical capabilities, with a commitment to ongoing adjustments as treatment progresses.

• Collaborate closely with a multidisciplinary team comprising physicians, occupational therapists, psychologists, and other mental health professionals to create and implement tailored rehabilitation programs.

• Maintain comprehensive case files and records, ensuring precise documentation of client information, eligibility criteria, insurance details, provided services, and ongoing progress notes.

• Monitor client progress diligently to evaluate the effectiveness of treatment and make necessary modifications to meet evolving client needs.

• Stay engaged with clients during their participation in job training and occupational placement programs, troubleshooting any issues that arise and ensuring a positive experience.

• Work proactively with clients' families to identify effective strategies for support, balancing professional guidance with personal insight.

• Assess client needs and document reports promptly to facilitate timely interventions and referrals to necessary resources.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Millennia, your client population will consist primarily of individuals experiencing mental health and mood disorders. Qualifications\n

• A Master’s Degree in a relevant field such as counseling, social work, psychology, or marriage and family therapy.

• A valid and current Licensed Professional Counselor (LPC) licensure.
