Work Environment

This role is based onsite within a vibrant assisted living facility that caters to residents with diverse healthcare needs. You will collaborate closely with a multidisciplinary team of healthcare professionals, including nurses, social workers, and rehabilitation specialists. Your work hours are set from Monday to Friday, with flexible schedules available between 9 am to 5 pm or 8 am to 4 pm.

Key Responsibilities

• Conduct thorough assessments of patients presenting with varying medical conditions, alongside comprehensive physical examinations.
• Diagnose and manage acute, exacerbated chronic illnesses, and other episodic health issues in a timely manner.
• Develop and implement detailed care plans tailored to meet the individual needs of each patient.
• Administer diagnostic tests, interpret results, and prescribe appropriate medications, ensuring adherence to best practices in pharmacology.
• Engage in patient education, providing essential information about health maintenance, medication usage, and treatment options.
• Document patient care in electronic medical records and communicate effectively with collaborating physicians regarding any changes in patient management.
• Facilitate referrals to specialists and coordinate with other healthcare providers to optimize patient outcomes and continuity of care.

Patient Population / Client Focus

Your primary focus will be working with patients in the assisted living facility, who may present a spectrum of health challenges, from chronic diseases to acute care needs. Understanding the unique medical and emotional needs of elderly patients is crucial. You will be expected to foster trust and positive relationships with residents and their families, advocating for their health and well-being.

Qualifications

• Master's Degree in Nursing and National Board Certification as a Nurse Practitioner.
• An active Nurse Practitioner license from the state of New York is mandatory.
• A minimum of two years of clinical experience in a similar role or setting.
• Certification for prescriptive authority, with a robust understanding of pharmacology and the management of medications.
• Exceptional communication skills with the ability to convey complex information to patients and their families in an understandable manner.
• Proficiency in electronic medical records systems to efficiently document patient interactions and treatment plans.

Preferred Experience

Compensation and Benefits
