Key Responsibilities

• Conduct thorough evaluations by reviewing patients' medical histories, performing physical assessments, and recommending appropriate treatment plans based on individual needs and progress.
• Effectively communicate evaluation results and treatment options to patients and their families, actively involving them in the goal-setting process for rehabilitation.
• Design and implement custom treatment protocols tailored to each patient's presentation and progress, utilizing cutting-edge methods and maintaining adherence to evolving healthcare standards.
• Collaborate with other health professionals, adjusting treatment plans as necessary, and documenting changes in patient conditions to ensure continuity of care.
• Facilitate appropriate discharge planning and provide clear instructions for follow-up care, ensuring patients leave with a solid understanding of their recovery process.
• Maintain meticulous records of patient care activities, including thorough initial evaluations, treatment documentation, and daily progress notes reflective of individual patient journeys.
