Key Responsibilities\n

• Maintain and prepare electronic medical records in alignment with established departmental and health system policies, ensuring accuracy in documentation and compliance with standards.

• Conduct thorough psychosocial assessments of patients and their families, focusing on their social, emotional, and physical needs, and identify barriers to care that must be addressed.

• Collaborate with interdisciplinary teams to create, implement, and coordinate individualized psychosocial treatment plans, while facilitating referrals to specialized services and community resources.

• Provide crisis intervention, supportive counseling, and grief counseling aimed at helping families navigate the complexities of illness and health-related issues.

• Manage an independent caseload with varying levels of clinical acuity, ensuring each patient's needs are met through tailored treatment approaches.

• Engage in proactive case finding to identify individuals and families needing psychosocial support, attending team conferences and interdisciplinary rounds as necessary.

• Offer psychoeducation to patients and families to aid their understanding of diagnoses, treatment options, and disease management, ensuring a smooth transition from youth to adult services.

• Participate in quality assurance initiatives, contributing to improvements in social work practices and the development of organizational policies and procedures.

• Perform mental health diagnoses within the framework of treatment planning as required, ensuring patient-centered care delivery.

• Provide clinical supervision to Master of Social Work (MSW) students and interns, fostering their professional development and integration into the field.

Qualifications\n

• A Master’s degree in a behavioral science or a related field from an accredited institution is required.

• Possession of valid licensure (LMFT, LCSW, LPCC) to practice as a mental health counselor in California.

Compensation and Benefits\n
