Key Responsibilities\n

• Conduct comprehensive assessments to identify patients' psychosocial needs, guiding your recommendations for appropriate treatment paths, referrals, and interventions.

• Provide short-term counseling and crisis intervention, utilizing evidence-based therapies tailored to individual patient circumstances.

• Assist patients in navigating and completing necessary forms for entitlements, supplemental insurance, and other benefits, strategically aiming to decrease reliance on emergency assistance programs.

• Facilitate access to prescription cost assistance programs by connecting patients with resources supported by pharmaceutical companies and local community initiatives.
\n
• Act as an advocate for patients, effectively communicating and negotiating with community agencies to ensure they receive the necessary support and services.

• Maintain precise and up-to-date patient records, prepare reports, and create documentation as required for case management and continuity of care.

• Coordinate transportation assistance for patients when needed, ensuring they do not face barriers to receiving care.
\n
• Participate in clinical staff meetings, case conferences, and team collaborations that focus on optimizing patient outcomes and enhancing service delivery.

Patient Population / Client Focus\n

In this role, you will work with a diverse array of patients from various backgrounds and life experiences. Our client population includes individuals dealing with mental health disorders, trauma, substance use issues, and social challenges that may affect their mental well-being. Qualifications\n

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of an active and independent license as a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• At least two (2) years of professional experience in a mental health counseling or therapeutic setting.

Preferred Experience\n

While not mandatory, experience in community health settings, familiarity with various treatment modalities, and proficiency in telehealth practices will be advantageous. Compensation and Benefits\n
