Position Overview

Heartfelt Connections Therapy is actively seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in a fully remote role, focusing on the unique needs of clients at our Detox Center. As a key member of our interdisciplinary team, you will leverage your specialized training and experience to provide essential support to individuals on their journey toward recovery. Your role will primarily involve assessing psychosocial needs and coordinating therapeutic services to foster holistic well-being.

Work Environment

This position allows you to work remotely, offering flexibility while impacting clients' lives from the comfort of your own home. You will collaborate closely with a diverse team of healthcare professionals, including psychiatrists, addiction specialists, and case managers, all working together to create a seamless continuum of care. While the primary work schedule is structured from Monday to Friday, 8:00 AM to 4:30 PM, occasional evening or weekend hours may be required to meet client needs and ensure consistent support.

Key Responsibilities

• Conduct comprehensive evaluations of clients' psychosocial needs, integrating emotional, social, and mental health factors into your assessment.
• Collaborate with clients and their families to tailor individualized care plans, ensuring they align with the clients’ goals and realities.
• Provide counseling and emotional support to clients dealing with the challenges of addiction and recovery, helping them navigate life transitions effectively.
• Advocate for clients by facilitating access to necessary resources and support services, ensuring they receive appropriate care throughout their recovery journey.
• Work integratively with other professionals in the Detox Center to coordinate services and optimize client care outcomes.
• Engage in regular follow-ups and monitor client progress, continually assessing needs and modifying care plans as necessary.
• Promote communication and assist in conflict resolution among family members to help build a supportive recovery environment for clients.
• Educate clients about available community resources, support groups, and programs designed to enhance their recovery and well-being.
• Maintain accurate documentation of client interactions, assessments, and care plans according to best practices in electronic health record (EHR) management.
• Stay updated on social work practices, addiction treatment methodologies, and compliance with relevant healthcare regulations and ethical standards.

Patient Population / Client Focus

In this role, your primary focus will be on individuals experiencing substance use disorders, many of whom are navigating the complexities of addiction, recovery, and co-occurring mental health issues. You will engage with a population that varies in age, background, and severity of condition, requiring you to adapt your approach to meet diverse needs while providing consistent, person-centered support.

Qualifications

• A Master’s degree in Social Work (MSW) from an accredited institution is required.
• A current and valid state licensure as a Clinical Social Worker (LCSW) is mandatory.
• A minimum of two years of professional experience in a social work capacity, ideally focused on addiction, mental health, or community service settings.
• Strong understanding of social work principles, addiction-related issues, and available community resources.
• Proficient in client assessment, counseling methodologies, and problem-solving techniques.
• Ability to empathize deeply, demonstrating compassion and building rapport with clients from diverse backgrounds.
• Excellent communication skills to collaborate effectively with both clients and interdisciplinary team members.
• Comfortable with electronic health record (EHR) systems and other relevant technology platforms.

Preferred Experience

• Prior experience working in a Detox Center or similar recovery-focused environment.
• Familiarity with trauma-informed care practices and motivational interviewing techniques.
• Experience in crisis intervention and conflict resolution strategies.

Compensation and Benefits

The compensation package for this position ranges from $60,000 to $100,000 annually, based on experience and qualifications.
