Key Responsibilities

In your role as a Licensed Clinical Social Worker, you will undertake a variety of key responsibilities, including:

• Acting as a liaison between clients, their families, and outpatient providers to ensure a cohesive care strategy.

• Communicating with insurance companies to facilitate pre-certification and manage utilization of services, ensuring financial barriers do not obstruct client care.

• Contributing to vital discharge and aftercare planning, focusing on creating personalized transition strategies that enhance recovery outcomes.

• Providing therapeutic meal support by modeling and promoting a healthy relationship with food, crucial for individuals in recovery.

• Delivering empathetic, safe, and respectful care tailored to each client's unique needs, while considering their intersectional identities.

Patient Population / Client Focus

Your expertise will primarily impact clients undergoing detoxification processes, including a diverse population spanning various ages and backgrounds. You will engage with clients facing substance use disorders, mental health challenges, and life transitions. Understanding and addressing the unique cultural, social, and personal factors affecting their treatment will be integral to your success.

Qualifications

The ideal candidate will possess the following qualifications:

• A Master's degree in Social Work from an accredited institution.

• A valid LCSW license in the state of Pennsylvania.

• Proven experience working with older adults, ideally in a mental health or substance use context.

Preferred Experience

We seek candidates with strong clinical and diagnostic skills, excellent communication abilities, and the capacity to work independently while also collaborating effectively within a team. Familiarity with electronic health records (EHRs) and case management software will be advantageous to streamline client interactions and record-keeping.

Compensation and Benefits
