Key Responsibilities\n

• Monitor, document, and communicate patient progress utilizing electronic medical records, ensuring accurate clinical handoffs for at-risk patients.

• Utilize clinical tools and technology to identify patients at risk of readmission, developing targeted interventions to mitigate these risks.

Patient Population / Client Focus\n

This position involves working with a diverse patient population facing a variety of mental health challenges, including but not limited to anxiety, depression, trauma-related disorders, and the psychosocial impacts of serious medical conditions. Qualifications\n

• A master's or doctorate degree in clinical psychology, counseling, or clinical social work from an accredited program (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD).

• An independent license in the state of residence (e.g., LCSW, LMFT, LPC) is required.

• Demonstrated ability to effectively manage a sizable caseload while interfacing positively with large providers and support staff.

Compensation and Benefits\n
