Work Environment\n

This position offers a remote work setting, allowing for flexibility while maintaining productivity and connection with our clients and team. As part of the Hospice Center’s extended care team, you will interface with other healthcare professionals to ensure seamless service delivery. Key Responsibilities\n

• Conduct thorough evaluations and assessments to identify the unique needs and strengths of each child and family.
\n
• Engage in the development and implementation of Individualized Family Service Plans (IFSP), tailoring each plan to support the specific goals and objectives of the child’s development.
\n
• Monitor outcomes and provide feedback as an integral member of a multidisciplinary team aimed at delivering the highest quality of care.
\n
• Develop strong rapport with children and families, fostering trust and open communication to create a healthy learning environment.
\n
• Provide education and support to families, equipping them with the knowledge and skills necessary to enhance their child's development.
\n
• Utilize evidence-based practices and Natural Learning Environment Practices in your interactions and service delivery.
\n
• Ensure compliance with all relevant federal and state regulations to uphold the health and safety standards for clients and staff.
\n
• Adapt strategies to meet the varied learning styles and needs of each child, guiding them through their developmental journey.

Patient Population / Client Focus\n

This role focuses primarily on infants and toddlers with disabilities or delays, providing essential early intervention services that span physical, cognitive, social/emotional, and communication skills. By working closely with families, you facilitate a supportive environment that promotes the child’s growth and independence during their critical early years of development.

Qualifications\n

• A Master’s Degree in Social Work or a Doctorate in Social Work is required.

• Current LCSW licensure by the Department of Public Health is mandatory.
\n
• A valid driver’s license is needed for any required travel.
\n
• One year of experience specifically working with children in early intervention settings is essential.

• Ability to demonstrate a commitment to employing Natural Learning Environment Practices in your approach.
\n
• Strong organizational and time management skills to effectively handle a caseload.
\n
• Exceptional communication skills, with the capacity to collaborate effectively within a team setting.

• Bilingual capabilities are highly encouraged to better serve our diverse client population.

Preferred Experience\n

While a minimum of one year of relevant experience is required, candidates with additional experience in early childhood education, family dynamics, or interdisciplinary teamwork are preferred. Experience using coaching models and natural environment teaching strategies will be advantageous.

Compensation and Benefits\n
