Work Environment

This role allows for remote work flexibility, providing an excellent work-life balance. Counselors can work from home while being connected to an enthusiastic team focused on collaborative care. In addition to remote work capabilities, some responsibilities may require attendance at our outpatient office in Erie, Pennsylvania, fostering a hybrid work environment that adapts to the needs of our staff and patients.

Key Responsibilities

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues effectively.
• Conduct initial patient intakes that emphasize thorough diagnostic and functional evaluations, making informed recommendations for treatment planning.
• Implement brief, focused therapeutic interventions tailored to patients requiring mental health support.
• Assess which patients benefit best from primary care services versus those needing specialized mental health treatment and facilitate appropriate referrals.
• Initiate and maintain follow-up communications with patients to assess their progress and adjust treatment plans as necessary.
• Develop and implement relapse prevention plans when indicated, assisting patients in maintaining stability in their daily lives.
• Monitor and coordinate behavioral health service delivery, ensuring seamless interactions with both internal and external treatment providers.
• Document detailed progress notes and diagnostic information in treatment charts to maintain accurate patient records.
• Engage in proactive consultations with primary care teams to ensure patient needs are met and to create comprehensive treatment plans.
• Empower patients with self-management skills and educational resources, encouraging active participation in their recovery journeys.
• Facilitate opening dialogues around healthy lifestyle choices that may affect both mental and physical health.
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals, ensuring the highest standard of care at all times.
• Perform additional duties as assigned, contributing to a team-oriented environment.

Patient Population / Client Focus

Our center serves a diverse patient population ranging from children to adults, addressing various mental health needs, including anxiety, depression, and trauma-related issues. As an LPC at Thrive Hard, you will be instrumental in providing care to individuals facing significant social challenges that impact their mental wellbeing. Qualifications

• A Master’s Degree in Clinical Counseling is required.
• Current licensure as a Licensed Professional Counselor (LPC) is mandatory.
• Comprehensive knowledge of mental health assessment techniques, treatment planning frameworks, and therapeutic modalities is essential.
• Proficiency in bilingual English-Spanish communication is preferred but not required.
• Strong understanding of the social and psychological effects of mental health conditions on individuals and families.
• Familiarity with community resources, their organization, and their effective utilization is beneficial.
• Ability to independently assess patients’ needs and recommend appropriate interventions.

Preferred Experience

• Experience in a multidisciplinary healthcare setting is advantageous.
• Background working with diverse populations and addressing various cultural considerations in patient care is a plus.
• Exposure to integrated care models, especially within federally qualified health centers (FQHC), is preferred.

Compensation and Benefits

This role offers a competitive salary ranging from $87,000 to $105,000 annually, commensurate with experience and qualifications. In addition to base pay, we provide a SIGN-ON BONUS as an incentive for new hires.
