Key Responsibilities

• Conduct comprehensive psychosocial assessments to diagnose and treat a spectrum of mental health issues, including anxiety disorders, depression, and behavioral problems.
• Develop and implement individualized treatment plans that align with each client’s unique needs, making necessary adjustments based on their progress and feedback.
• Facilitate referrals to additional services and resources, ensuring that clients have access to a comprehensive support system that may include community resources, psychiatrists, and support groups.
• Work collaboratively with multidisciplinary teams to create effective discharge plans, enhancing continuity of care and follow-up support for clients transitioning from treatment.
• Ensure all clinical activities, documentation, and client interactions are conducted in adherence with current regulations and standards, including the Mental Health Practice Act.
• Prioritize and organize daily tasks based on client needs, handling crisis situations as they arise and facilitating timely interventions.
• Provide education and support resources to clients and their families, aiding them in understanding mental health issues and available treatments.
• Advocate effectively for clients to foster seamless communication with care teams and outside agencies when necessary.
• Utilize the telehealth platform for case management—writing notes, scheduling appointments, and tracking client progress all within a single app.

Qualifications

Compensation and Benefits
