Key Responsibilities\n

• Ensure thorough clinical handoffs for at-risk patients, co-ordinating referrals and resources as needed.

• Document and communicate patient progress using electronic medical records, keeping track of treatment goals and social work plans.

• Collaborate with an interdisciplinary team to anticipate patient needs and enhance the effectiveness of care plans.

• Maintain current knowledge of government and private insurance networks to assist with patient transitions.

Patient Population / Client Focus\n

Your primary focus will be on patients suffering from acute to chronic mental health conditions exacerbated by physical health challenges. This includes individuals with terminal illnesses, their families, and caregivers. Qualifications\n

• A Master's or Doctorate degree in clinical psychology, counseling, or clinical social work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD), conferred by an accredited institution.

Compensation and Benefits\n
