Position Overview

Swift Peace Physical Therapy is actively seeking compassionate and skilled Full-Time Physical Therapists to deliver exceptional in-home care to our patients in Fairfax, Virginia. In this role, you will evaluate, plan, and implement treatment programs for individuals recovering from surgery, injury, or managing chronic conditions, ensuring a holistic approach to their health and physical wellbeing.

Work Environment

Key Responsibilities

• Conduct thorough evaluations of patients to identify impairments and functional limitations, setting realistic goals and developing detailed treatment plans based on physician recommendations.
• Document all aspects of patient care, including evaluations, treatment progress, plans of care, and discharge summaries, ensuring compliance with skilled documentation standards.
• Deliver hands-on therapeutic interventions, utilizing various modalities and exercise programs that enhance patients' mobility and functional independence.
• Supervise and mentor Physical Therapy Assistants (PTAs) and support staff in delivering effective patient care, providing guidance and support to enhance team performance.
• Engage in regular communication with interdisciplinary teams to coordinate care, problem-solve, and adjust treatment plans as necessary to achieve optimal patient outcomes.
• Provide constructive feedback to the Director to contribute to a positive workplace culture, maintaining high productivity standards and fostering professional growth.

Patient Population / Client Focus

The patients you will serve range widely, from post-operative individuals to those with chronic conditions such as arthritis or neurological disorders. Our focus is on delivering tailored care that meets the unique needs of each patient, ensuring that every plan of care reflects their age, developmental level, and specific functional goals.

Qualifications

• Minimum of a Bachelor's Degree in Physical Therapy from an accredited university.
• Current and valid licensure as a Physical Therapist in the State of Virginia.
• Demonstrated ability to communicate effectively, both verbally and in writing, with patients, families, and professionals.
• Proficiency in using electronic health records and common office software is essential for documentation and communication.

Preferred Experience

While a minimum of one year of experience in a clinical setting is required, we particularly welcome candidates with prior home care experience, as this knowledge can significantly enhance patient engagement and care delivery. Experience working with a diverse patient population will also be beneficial.

Compensation and Benefits

Swift Peace Physical Therapy offers a competitive salary ranging from $80,000 to $100,000 annually, commensurate with experience.
