Work Environment

The work setting for this position is exclusive to onsite home care, where you will be visiting patients in their residences. You will work closely with interdisciplinary teams, including nurses, occupational therapists, and case managers, to provide a holistic care approach that is vital in home settings.

Key Responsibilities

• Formulate and direct personalized physical therapy treatment plans aimed at revitalizing orthopedic functions, preventing disabilities, and alleviating pain based on initial evaluations and ongoing assessments.
• Oversee and guide physical therapy assistants to ensure precise execution of care strategies and treatment protocols.
• Conduct comprehensive evaluations utilizing diagnostic tests to assess clients' muscle, nerve, and joint functions as well as their ability to perform daily activities.
• Analyze client home environments to recommend adjustments that could enhance rehabilitation and independence.
• Administer prescribed physical therapy treatments, including therapeutic exercises, manual therapies, and modalities such as traction and massage.
• Evaluate and adjust the use of prosthetic and orthotic devices, making modifications as informed by progressive evaluations.
• Collaborate in the interdisciplinary team for case conferences, contributing to comprehensive treatment plans and monitored outcomes.
• Document clinical records meticulously, including evaluations, treatment plans, progress notes, and discharge summaries, ensuring compliance with health regulations.
• Provide education to clients and their families regarding physical therapy devices, exercises, and overall health maintenance to empower them towards self-care.

Patient Population / Client Focus

This role specifically targets clients requiring orthopedic rehabilitation, which may include post-surgical patients, individuals recovering from injuries, and those with chronic musculoskeletal conditions. Our clients span various ages but primarily consist of adults seeking to regain their mobility and independence in their everyday lives.

Qualifications

• Graduate of an accredited physical therapy program with a Bachelor's Degree.
• A minimum of one year of professional experience as a physical therapist in either acute or home care settings.
• Current and valid Physical Therapy licensure in the state of Florida, in good standing.
• Current CPR certification.
• A valid driver's license and reliable automobile insurance to travel between client locations.

Preferred Experience

• Experience in home healthcare with a focus on orthopedic rehabilitation is a strong advantage.
• Familiarity with OASIS documentation and Medicare guidelines is preferred.
• A history of effective collaboration within interdisciplinary care teams.
