Work Environment

This role is structured to take place directly in our patients' homes, allowing you to create a comfortable and personalized environment for rehabilitation. Key Responsibilities

• Conduct thorough examinations and evaluations of patients to assess their physical status and rehabilitation needs.
• Create tailored treatment plans that address specific mobility goals and align with the continuum of care for each individual patient.
• Monitor patient progress, modify treatment approaches, and set new goals as needed to ensure the best functional outcomes.
• Educate patients and their families about the recovery process, treatment plans, exercises, and safe practices to facilitate home rehabilitation.
• Regularly re-evaluate patient conditions and adjust treatment strategies based on the latest clinical evidence and patient feedback.
• Document patient information accurately to support care continuity and ensure compliance with healthcare regulations.
• Utilize and maintain specialized physical therapy equipment while ensuring safety and effectiveness in patient applications.
• Adhere to state practice acts and organizational policies to uphold the highest standards of professional practice.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient demographic, primarily those recovering from orthopedic surgeries, injuries, and chronic conditions affecting mobility. Qualifications

Education/Training: Candidates must hold a Bachelor's degree or higher from an accredited physical therapy program. Relevant advanced degrees are encouraged.

Licensure/Certification:

• Must possess an active Physical Therapist license issued in the State of Florida. A temporary permit is acceptable for new graduates.
• Current BLS/HealthCare Provider certification is required for all applicants.

Preferred Experience

Prior experience in home health care or a similar setting is highly desirable, as it enhances your ability to navigate the challenges of delivering care in non-traditional environments.
