Work Environment

As a Physical Therapist at our rehabilitation center, you will practice in a vibrant and patient-centered setting. Key Responsibilities

• Conduct thorough patient examinations and evaluations to develop effective treatment plans addressing individual functional limitations and disabilities.
• Utilize your knowledge of state practice acts to ensure compliance with local regulations and understand the scope of practice for physical therapists.
• Implement and adapt treatment protocols based on evaluation findings and patient progress, remaining flexible in response to the evolving healthcare landscape.
• Document all patient care activities meticulously—including initial evaluations, treatment plans, daily encounter notes, progress summaries, and discharge evaluations—to provide a comprehensive overview of the patient's treatment trajectory.
• Collaborate seamlessly with other healthcare professionals to refine treatment plans, while responding appropriately to changes in patient conditions that may necessitate adjustments in care.
• Lead discharge planning and formulate follow-up care recommendations that ensure continuity and quality of care after patients leave the facility.
• Engage in facility meetings to contribute to team discussions focused on improving patient access and participation in the Access NRH initiatives, aimed at removing barriers to care.

Patient Population / Client Focus

The Physical Therapist role at Hype Motion Physical Therapy, LLC primarily involves working with patients who have sustained musculoskeletal injuries, post-surgical rehabilitation requirements, or chronic pain conditions. Our diverse patient population includes individuals of all ages, each requiring personalized attention and care strategies to promote recovery and regain functionality. Qualifications

• Hold a Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.
• Possess a valid state license as a Physical Therapist from the Physical Therapy Examining Committee.
• A minimum of two years of outpatient orthopedic experience, demonstrating your ability to thrive in a fast-paced environment.
• Current CPR certification indicating readiness for providing emergency care when necessary.

Preferred Experience

Experience in utilizing electronic medical records (EMR) systems and familiarity with clinical assessment tools will be advantageous, allowing you to enhance patient documentation and treatment tracking. Any specialty training or certifications in manual therapy or sports rehabilitation is also highly regarded.
