Orthopedic Physical Therapist - Full-Time Home Care Specialist

Work Environment

As a Physical Therapist at Life Mission Nursing and Rehab, you will work directly with patients in their home environments, allowing you to assess their daily living conditions and tailor treatments accordingly. Key Responsibilities

• Conduct comprehensive evaluations of orthopedic conditions, utilizing patient medical histories and physical assessments to establish diagnoses.
• Develop and implement individualized treatment plans tailored to each patient's goals, preferences, and specific conditions.
• Apply hands-on therapeutic modalities, including manual therapy techniques, joint mobilization, and tailored therapeutic exercises to promote recovery.
• Educate patients and their families on effective injury prevention tactics, proper ergonomics in daily activities, and home exercise programs to support rehabilitation.
• Track and document patient progress meticulously, adjusting treatment plans in response to objective measures and ongoing patient feedback.
• Collaborate with a multidiscipline team of healthcare professionals, including physicians and nursing staff, to ensure comprehensive care for patients.
• Utilize advanced equipment and therapy modalities while staying informed about the latest advancements in orthopedic physical therapy practices.
• Maintain timely and accurate documentation of all patient interactions, ensuring compliance with regulatory standards and organizational protocols.
• Participate in continuous professional development, contributing to team meetings, case reviews, and ongoing training opportunities to enhance clinical skills.

Patient Population / Client Focus

This role emphasizes treating a diverse array of patients, primarily focusing on individuals with orthopedic conditions such as fractures, post-surgical rehabilitation, joint replacements, and chronic pain conditions. Qualifications

• Bachelor's Degree in Physical Therapy or higher (Doctor of Physical Therapy preferred) from an accredited program.
• A valid California Physical Therapy License or eligibility to obtain licensure in California.
• Strong clinical background in orthopedic physical therapy, with demonstrated competence in manual therapy techniques.
• Excellent assessment skills, with the ability to diagnose and create treatment plans for a range of musculoskeletal conditions.
• Strong interpersonal and communication skills, essential for engaging and educating patients.
• A demonstrated ability to work collaboratively within a multidisciplinary team.
• Familiarity with physical therapy management software for patient scheduling and documentation.
• Prior experience in outpatient orthopedic settings or sports rehabilitation is highly desirable.

Preferred Experience

• Experience with electronic health records (EHR) systems.
• Board Certification in Orthopedic Physical Therapy (OCS) is a plus.
• Prior experience in home healthcare settings or knowledge of community healthcare resources.

Compensation and Benefits

We offer a competitive salary range of $100,000 to $125,000 annually, commensurate with experience, alongside a comprehensive benefits package.
