Work Environment

This onsite position primarily takes place in the homes of our clients across Long Beach and surrounding areas. The home care setting allows therapists to evaluate and address patients' needs in the environment where they perform daily activities, leading to more effective and practical rehabilitation solutions. Key Responsibilities

• Conduct comprehensive assessments of patients to determine their physical therapy needs and develop personalized treatment plans.
• Administer physical therapy interventions, including exercises, massage, and stretching techniques, to enhance patient recovery and mobility.
• Guide patients through essential functional activities like bed mobility, safe transfers, gait training, and balance activities, ensuring a focus on safety and effectiveness.
• Monitor and document patient progress meticulously, noting any changes in their condition before, during, and after therapy sessions, and communicate these updates to supervising physical therapists.
• Instruct patients and their families on post-therapy care, providing educational resources and support to help maintain patient gains.
• Collaborate regularly with the clinical team and engage in mentoring opportunities to share best practices and enhance the skills of staff members.

Patient Population / Client Focus

Our patient demographic includes individuals recovering from various orthopedic conditions, surgeries, and injuries. This may range from post-operative rehabilitation following joint replacements to recovery after sports-related injuries. Many patients are elderly with unique mobility challenges, requiring specialized attention and customized solutions to ensure they can regain independence and quality of life.

Qualifications

• A Bachelor's Degree in Physical Therapy from a CAPTE-accredited program.
• Current California Physical Therapy Assistant License (PTA) and relevant degree.
• Minimum of 2 years of experience in orthopedic physical therapy, ideally in a home care setting.
• Exceptional communication skills, allowing for clear interaction with patients and effective collaboration with the healthcare team.
