Key Responsibilities

• Conduct thorough evaluations of patients, integrating comprehensive medical histories and specific assessment techniques to determine individualized treatment plans.
• Communicate evaluation findings clearly to patients and their families, involving them in goal-setting to enhance treatment engagement and adherence.
• Implement targeted treatment protocols derived from evaluation results, continuously adapting methods based on patient progress and evolving healthcare standards.
• Collaborate with a multidisciplinary team, consulting with other healthcare professionals as necessary and documenting any changes in patient conditions that require adjustments to care plans.
• Ensure meticulous completion of discharge planning, providing follow-up care instructions and timely documentation that captures each patient's journey through treatment.
• Manage time effectively to meet productivity benchmarks while maintaining an accurate schedule of appointments and being punctual for meetings.
• Participate in continuous quality improvement initiatives to help the clinic achieve its objectives and comply with regulations set forth by accrediting bodies.

Qualifications

Compensation and Benefits
