Key Responsibilities

• Perform thorough evaluations by reviewing patients' medical histories, conducting specific assessments, and delivering accurate documentation of findings and treatment recommendations.
• Communicate evaluation results and develop treatment plans in partnership with patients and their families, ensuring their involvement in goal-setting processes.
• Effectively implement treatment protocols that are customized to individual patient presentations while adhering to evolving healthcare standards.
• Collaborate with a multidisciplinary team, recognizing and documenting any changes in patient conditions that necessitate adjustments to the care plan or additional medical consultations.
• Facilitate comprehensive discharge planning that includes follow-up care, ensuring that documentation of patient interventions is meticulous, timely, and reflective of each patient's ongoing progress.
• Manage time efficiently to meet productivity goals while maintaining a consistent schedule and timely attendance at meetings.
• Contribute proactively to departmental goals and objectives, while actively adhering to state regulations and broader accreditation requirements.
• Pursue ongoing education for professional growth and organizational development, potentially serving as a clinical instructor for students in training.
• Promote an exceptional customer experience by demonstrating flexibility and responsiveness to patient needs and preferences.
• Participate actively in initiatives aimed at removing barriers to patient access, making sure to utilize site-specific data to improve service delivery.

Patient Population / Client Focus

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.

Preferred Experience

Compensation and Benefits
