Key Responsibilities\n

• Patient Assessment: Conduct comprehensive assessments to evaluate the mental health needs of clients, tailoring therapeutic strategies accordingly. This involves ongoing evaluations that adapt to clients’ evolving needs.

• Progress Communication: Facilitate communication among team members, families, and other relevant parties regarding the clients’ progress, ensuring all stakeholders are informed.

• Appointment Coordination: Develop a tentative schedule for client sessions, home visits, and observation periods to closely monitor therapeutic advancements.

• Program Implementation: Ensure that all treatment plans are effectively executed and adjusted as necessary based on client feedback and outcomes.

• Team Participation: Actively engage in Interdisciplinary Team Meetings to discuss cases and collaborate on treatment strategies.

• Documentation: Regularly complete progress notes to track advancements and challenges, ensuring that essential information is systematically recorded and accessible.

• Training and Support: Organize training sessions to enhance team competency in program implementation and documentation standards, fostering a culture of continuous improvement.

• Follow-Up: Conduct follow-up interviews and home visits to provide uninterrupted support, ensuring that clients have the resources they need for lasting success.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related mental health field is required.

• Possession of an active and unrestricted LPC license in the state of Arizona.

• Minimum of two years of counseling experience, ideally in a clinical setting.
