Key Responsibilities\n

• Conduct comprehensive assessments of patients to understand their mental health needs and develop appropriate treatment plans.

• Maintain open lines of communication with supervisors, family members, and other stakeholders involved in the patient's care to monitor progress and adjust treatment plans as necessary.

• Create and manage a tentative schedule of client appointments, home visits, and therapeutic observations to ensure effective case management.

• Conduct home visits as required to gather data firsthand through observations and interviews, ensuring a holistic understanding of the patient's environment.

• Ensure that the implementation of the treatment program is executed effectively, adhering to established therapeutic practices.
\n
• Participate actively in interdisciplinary team meetings to share insights and contribute to case discussions, enhancing collaborative care quality.

• Document monthly and quarterly progress notes that are essential for tracking patient improvement and necessary changes in treatment strategies.

• Organize and lead training for staff to ensure they are adequately equipped to implement program plans and maintain high standards of documentation.

• Provide ongoing support through follow-up interviews and necessary home visits to reinforce therapeutic strategies and adjust care plans as needed.

• Serve as a liaison with the public and mental health community to foster support and promote developmental goals in mental health care practices.

• Contribute insights towards developing training materials that enhance the program and support the personal and professional growth of individuals.

Patient Population / Client Focus\n

This role will primarily focus on diverse patient populations, including both adults and youth. Our counselors work effectively with individuals facing a variety of challenges, including anxiety, depression, trauma, and substance misuse. The LPC will play a crucial role in providing tailored mental health care that meets the unique needs of each client.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential.

• Candidates must possess active, unrestricted licensure as a Professional Counselor (LPC) in the state of New Jersey.

• A minimum of 2 years of clinical counseling experience, demonstrating a proven ability to work effectively with various client populations.

• Experience and specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse interventions are highly preferred.

• Strong understanding of legal and ethical standards governing the mental health profession.

• Exceptional interpersonal and communication skills for establishing rapport with clients and stakeholders.

• Proficiency in collaborating seamlessly within a healthcare team environment.

• Familiarity with Electronic Medical Records (EMR) software and related technologies is required.
