Key Responsibilities

• Conduct comprehensive evaluations to assess patient impairments, functional limitations, and rehabilitation needs, using diagnostic tests and functional assessments.
• Develop and execute individualized treatment plans in accordance with the overall care strategy, focusing on mobility improvement, pain relief, and functional independence.
• Administer therapeutic modalities, including manual therapy, therapeutic exercises, and assistive technology, ensuring adherence to prescribed protocols.
• Oversee and mentor Physical Therapy Assistants, ensuring they carry out treatment plans effectively and safely, while adhering to state regulations.
• Regularly monitor and document patients' progress, adjusting treatment plans as necessary to optimize recovery outcomes.
• Engage with patients and their families to provide education on home exercises, pain management strategies, and the utilization of assistive devices.
• Collaborate as a member of the interdisciplinary team, participating in case conferences, contributing to care plan development, and discussing patient progress.
• Maintain high-quality clinical records including evaluations, treatment notes, progress reports, and discharge summaries, ensuring compliance with regulatory standards.
• Evaluate home environments for potential rehabilitation challenges and offer recommendations to enhance patient independence.
• Contribute to continuous quality improvement initiatives to enhance patient care and service delivery.

Patient Population / Client Focus

The Physical Therapist will primarily work with a diverse patient population, including those recovering from surgical procedures, managing chronic illness, or rehabilitating post-injury. The ability to engage and adapt treatment approaches to meet the needs of a varied clientele is essential to promote recovery and improve patient outcomes.

Qualifications

• A Bachelor's Degree in Physical Therapy or related field, with a strong foundation in musculoskeletal and rehabilitative principles.
• Active Physical Therapist license in the State of New Jersey with a clean record free from sanctions.
• Exceptional skills in patient assessment and physical therapy modalities, with an aptitude for applying evidence-based techniques.
• Proficiency in using electronic medical records (EMR) software, particularly for documenting care processes and patient interactions.
• Strong interpersonal and communication skills to effectively educate and motivate patients and families.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy setting, preferably within a rehabilitation center or hospital environment.
• Proven experience with various manual therapy techniques, therapeutic exercises, and functional mobility training.

Compensation and Benefits
