Work Environment

This position is onsite in patients' homes across Lakewood and surrounding areas. Working in home care allows our Physical Therapists to provide direct, personalized treatments in a familiar and comfortable setting for patients. Key Responsibilities

• Conduct comprehensive evaluations of patients to identify their physical therapy needs and develop personalized treatment plans aimed at maximizing their functional independence.
• Utilize various therapeutic approaches including manual therapy, exercise regimens, and assistive technologies to alleviate pain and enhance mobility.
• Regularly document patient progress, treatment outcomes, and any modifications to care plans to ensure clarity and consistency in care delivery.
• Provide education and support to patients and their families regarding treatment plans and the use of therapeutic equipment to facilitate at-home recovery.
• Collaborate effectively with multidisciplinary teams, including nurses and occupational therapists, to optimize patient care and address complex medical issues.
• Stay abreast of innovations in physical therapy practices by engaging in continuing education workshops and professional development activities.
• Maintain clear and professional communication with patients, families, and other healthcare providers through verbal and written means, ensuring that patient needs are met and understood.

Qualifications

• Minimum of a Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state license and registration in New Jersey to practice physical therapy.

Compensation and Benefits
