Key Responsibilities

As a Licensed Clinical Mental Health Counselor, your responsibilities will include:

• Conducting comprehensive assessments to identify the specific needs and challenges of your patients, ensuring tailored treatment plans that align with their goals.
• Facilitating open communication among supervisors, direct care staff, and family members to provide updates on patient progress and collaborate on care strategies.
• Designing and maintaining a tentative schedule of appointments, home visits, and observations to track progress and enhance accountability.
• Coordinating home visits and in-home observations as necessary, including chart reviews and staff interviews to fully understand individual circumstances.
• Implementing program plans effectively and ensuring they meet compliance and quality standards.
• Participating actively in Interdisciplinary Team Meetings, contributing insights from your clinical expertise to improve overall patient care.
• Completing monthly and quarterly progress notes, documenting the ongoing development of therapy goals and objectives.
• Organizing training sessions for staff and team members to ensure familiarity with program plans and accurate documentation practices.
• Providing consistent follow-up and support through interviews and additional home visits tailored to meet evolving patient needs.
• Maintaining thorough records to facilitate easy access to patient information for all relevant stakeholders.
• Acting as a liaison within the mental health community to foster partnerships that further our clinic’s developmental and outreach goals.
• Assisting with the creation of training materials to enhance the effectiveness of programs for individuals seeking assistance.

Patient Population / Client Focus

This role allows you to work with a diverse array of client demographics, focusing primarily on individuals struggling with mental health issues, including but not limited to anxiety, depression, trauma, substance abuse, and relationship challenges. Your ability to provide compassionate, evidence-based care to both adults and youth will be essential in achieving positive patient outcomes.

Qualifications

To be considered for this position, applicants must meet the following qualifications:

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.
• An active and unrestricted license as a Professional Clinical Counselor (LPCC) in the state of California.
• Minimum of two years of counseling experience, particularly in a clinical context.
• A strong understanding of mental health legal and ethical standards.
• Demonstrated excellence in interpersonal and communication skills, enabling you to build rapport with clients from diverse backgrounds.
• Proven experience working collaboratively within a healthcare team.

Preferred Experience

Ideal candidates will possess a specialization in family therapy, couples therapy, trauma-informed care, or substance misuse interventions. Experience with EMR software and related technologies will also be advantageous, enhancing your efficiency in patient management and documentation.

Compensation and Benefits

Cooper Care Clinic offers a competitive salary range of USD 90,000 to 100,000 per year.
