Key Responsibilities\n

• Proactively engage with clients, utilizing evidence-based counseling techniques to prevent crises and minimize reliance on emergency services.

• Facilitate smooth transitions between hospital and outpatient services, collaborating with hospital staff to streamline discharge processes and support ongoing care.

• Document all interactions meticulously within our electronic health records, maintaining up-to-date client files and tracking treatment progress.

• Attend regular team meetings to discuss case management strategies and share insights on improving patient engagement and outcomes.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling.

Compensation and Benefits\n

The salary for this position ranges from $100,000 to $105,000 annually , based on experience and caseload management.
