Key Responsibilities

• Conduct thorough evaluations of patients in their homes to assess their physical condition, limitations, and rehabilitation needs.
• Develop personalized treatment plans that incorporate a variety of approaches including physical exercises, manual therapy, and technological aids to relieve pain and enhance mobility.
• Utilize evidence-based practices and the latest therapeutic techniques to maximize patient recovery and functionality.
• Document all treatment plans, interventions, and patient progress meticulously to ensure continuity of care and compliance with healthcare standards.
• Regularly review and update therapeutic goals to ensure that patients are continually progressing towards their rehabilitation objectives.
• Provide education and training to patients and their families regarding condition management, use of assistive devices, and home exercise programs.
• Engage in ongoing professional development activities such as attending workshops, participating in relevant courses, and networking within the physical therapy community to remain current on best practices and emerging trends in rehabilitation.

Patient Population / Client Focus
