Work Environment

This position revolves around providing therapy in the home care environment, allowing our therapists to engage closely with patients in familiar settings. You will interact regularly with patients and their families, healthcare providers, and our supportive in-house clinical team. Key Responsibilities

• Conduct thorough initial evaluations of patients, which include reviewing medical histories, assessing physical abilities, and determining treatment protocols based on evidence and patient needs.

• Design and implement individualized treatment plans that address specific orthopedic conditions, utilizing the latest methodologies and tools in the field.

• Continuously monitor and document patient progress, adjusting treatment plans as necessary to achieve set goals effectively.

• Engage in multidisciplinary consultations with healthcare professionals to coordinate comprehensive care, ensuring modifications to treatment plans are timely and appropriate based on patient status.

• Maintain accurate and timely documentation of treatment sessions, evaluations, and discharge planning in compliance with organizational and regulatory standards.

• Ensure compliance with state, local, and federal regulations by completing all required training and maintaining licensure as needed.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required to ensure advanced knowledge and skills in the practice.

• Current CPR certification to guarantee safety in emergency situations.

Preferred Experience

Compensation and Benefits
