Key Responsibilities\n

• Utilize our electronic medical record system to effectively monitor, document, and communicate patient progress towards their established goals.

• Engage with a multidisciplinary team, actively participating in discussions to anticipate patient needs and adjust care plans accordingly.

• Provide education and support to patients and their families regarding transition plans, offering resources that facilitate smooth transitions to different levels of care.

• Maintain up-to-date knowledge of government regulations and private payer services to ensure patients have access to the care and resources they need during transitions.

• Actively coordinate with community and health care resources, ensuring a seamless integration of care tailored to individual patient needs.

Patient Population / Client Focus\n

You will primarily work with a diverse patient base, which includes individuals coping with various mental health disorders, as well as those managing effects from physical health challenges. By engaging in telehealth, you will reduce barriers to access and enhance the quality of care for patients who may have difficulty attending in-person sessions.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling is required.

• An active professional license is necessary; candidates must possess either LMFT (Licensed Marriage and Family Therapist), LPC (Licensed Professional Counselor), or LCSW (Licensed Clinical Social Worker) credentials.

• Strong ability to manage a large caseload while maintaining a high level of patient care and attention.

Compensation and Benefits\n

The position offers a competitive salary range of $100,000 to $105,000 annually, calculated based on caseload and experience.
