Key Responsibilities\n

• Collaborate with a multi-disciplinary healthcare team to develop and implement individualized treatment plans, including follow-up discharge plans based on comprehensive psychosocial assessments.

• Establish and maintain professional relationships with community stakeholders, representing MindShift at various meetings to advocate for resources that address social determinants affecting our patients.

• Conduct crisis intervention sessions and assessments, specifically focusing on emotional and behavioral challenges faced by patients, ultimately guiding them towards recovery.

• Maintain open communication with medical professionals, providing updates on patient progress, addressing any concerns, and making necessary referrals for additional support services.

• Keep detailed, accurate clinical records and progress notes in compliance with department standards, ensuring that all documentation reflects the patient's treatment journey.

Patient Population / Client Focus\n

As a Licensed Mental Health Counselor at MindShift, you will primarily engage with a diverse population facing various mental health challenges. This includes individuals coping with anxiety, depression, and substance use disorders. Your compassionate and knowledgeable support will be crucial in creating an atmosphere conducive to recovery, allowing patients to heal emotionally, cognitively, behaviorally, and socially.

Qualifications\n

• A Master’s degree in Marriage and Family Therapy, Counseling, Social Work, Psychology, or a closely related field.

• A valid state-issued license as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).

• A minimum of three years of experience in an outpatient mental health setting, demonstrating strong clinical skills and familiarity with community health resources.

• Proven ability to foster professional relationships with peers, other medical professionals, and clients, focusing on the therapeutic alliance.

• Comprehensive understanding of various therapeutic modalities, assessment tools, and evidence-based practices.

• Exceptional communication skills, both verbal and written, to effectively convey clinical information and support patient insights.

Preferred Experience\n

Experience working with underserved populations and familiarity with integrated care models will be advantageous. Additionally, proficiency in trauma-informed care practices and familiarity with digital health platforms will enhance your efficacy in this role.

This role offers a competitive salary range of $100,000 to $105,000 annually, reflecting your experience and expertise in the field.
