Key Responsibilities

• Communicate effectively with physicians, detailing patient progress, address any concerns, and identify potential challenges in treatment plans.

• Consult with family members and support systems to coordinate comprehensive client treatment and develop effective discharge plans.

• Maintain accurate and timely documentation, including clinical notes and progress reports, in alignment with the organization’s policies and electronic medical record systems.

• Collaborate closely with multidisciplinary teams to develop treatment plans based on comprehensive psychosocial assessments, while implementing recognized therapeutic interventions.

• Facilitate client discharge planning, ensuring seamless transitions to community resources and services, and engage with various providers to enhance accessibility to ongoing care.

Patient Population / Client Focus

As a Licensed Mental Health Counselor at Regenerate Wellness Clinic, you will primarily work with a diverse population facing various mental health challenges, including anxiety, depression, trauma, and substance use disorders. Our clients encompass individuals from all walks of life, requiring a multifaceted approach to treatment. This role emphasizes cultural competence and understanding, as you will engage with clients from varied backgrounds and experiences, ultimately striving for improved mental health outcomes.

Qualifications

• A Master’s degree in Social Work, Counseling, Marriage & Family Therapy, or Psychology.

• Current active state licensure as an LCSW, LMFT, LPC, or equivalent.

• A minimum of three years of experience providing outpatient mental health services.

• Proficient computer skills are essential, including familiarity with electronic medical records (EMRs), word processing software, email communication, and spreadsheets.

• Demonstrated ability to work independently, showing strong organizational skills and self-direction in managing work tasks.

Preferred Experience

• Prior experience working within a community health clinic environment is highly desirable.

• Experience with telehealth platforms and remote counseling techniques, enhancing accessibility and engagement with clients.

• Knowledge of trauma-informed care practices and techniques for supporting clients in recovery-focused environments.

Compensation and Benefits
