Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations to inform treatment planning.

• Provide brief, focused therapeutic interventions for patients seeking mental health support and maintain effective treatment tracking.

• Collaborate with primary care providers to identify and manage mental health and psychosocial issues within the patient population.

• Document patient progress and assessment data diligently in treatment charts, ensuring accurate and timely records management.
\n
• Initiate and coordinate follow-up care, monitoring patient progress and making necessary adjustments to treatment plans as needed.

• Develop relapse prevention plans and support patients in maintaining stable functioning in their daily lives.
\n
• Connect patients with additional resources and treatment providers, safeguarding continuity of care in adherence with patient consent.

• Conduct consultations with clinic management and other healthcare team members, assisting in behavioral health program evaluation and enhancement.

• Educate patients on self-management techniques and lifestyle choices to empower them in their mental health recovery.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Current and valid Licensed Professional Clinical Counselor (LPCC) License.

Preferred Experience\n

• Experience in working within a Federally Qualified Health Center (FQHC) or similar community health model.

• Bi-lingual in English and Spanish is preferred but not mandatory.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
