Key Responsibilities\n

As a Licensed Professional Counselor at Conejos Community Health Care Center, your daily responsibilities will include:

• Managing a diverse caseload of clients and fostering professional relationships that promote trust and effective therapy.

• Conducting comprehensive intake assessments to evaluate the mental health needs and preferences of each client.
\n
• Developing, implementing, and revising person-centered treatment plans, adapting interventions based on clients' progress and changing needs.

• Maintaining accurate and timely documentation of client interactions and clinical outcomes to ensure compliance and facilitate quality care.

• Holding individual therapy sessions to address various mental health challenges and utilizing evidence-based modalities to drive positive outcomes.

• Regularly meeting with clients to monitor progress and stability, and preparing for appropriate discharge planning when necessary.

• Participating in collaborative discussions with multidisciplinary teams and external healthcare providers to enhance client care.

• Engaging in professional development activities and supervision to refine clinical skills and stay updated with best practices.

The LPC will primarily work with youth, addressing a wide range of mental health challenges, including anxiety, depression, behavioral issues, and trauma-related concerns. Qualifications\n

The ideal candidate will possess:

• A Master's Degree in Counseling, Psychology, Social Work, or a related field.

Compensation and Benefits\n
