Work Environment

As a Home Care Physical Therapist, you will operate primarily within a patients' home environment. Key Responsibilities

• Conduct comprehensive evaluations of patients recovering from injuries, surgeries, or illnesses to develop individualized treatment plans.
• Utilize a variety of therapeutic techniques, including exercise routines, manual therapy, and assistive devices to alleviate pain and restore functional mobility.
• Track and document patient progress meticulously, adjusting treatment methods and goals based on ongoing assessments.
• Educate patients and their families on rehabilitation procedures and the effective use of assistive modalities to promote independence.
• Foster strong communication and collaboration with caregivers and other health professionals to ensure a holistic approach to patient care.
• Continuously pursue professional development opportunities through workshops, training sessions, and peer networking to maintain cutting-edge knowledge in physical therapy practices.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure as a Physical Therapist.
