Key Responsibilities

Your day-to-day responsibilities as a Physical Therapist include:

• Conducting thorough initial evaluations to assess patients' physical abilities, limitations, and medical history.
• Developing comprehensive treatment plans that align with patients' personal health goals and optimizing their rehabilitation process.
• Offering consultations to coordinate care with physicians and other healthcare providers, ensuring a seamless continuum of care for patients.
• Educating patients and their families on conditions, treatment plans, and self-care techniques to enhance their understanding and participation in recovery.
• Maintaining accurate documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with legal and regulatory standards.
• Participating in community outreach activities to promote physical therapy services and improve public health awareness.
• Collaborating with other healthcare professionals, support staff, and interdisciplinary teams to deliver high-quality patient care.
• Attending staff meetings and engaging in continuous professional development through education and self-study.

Patient Population / Client Focus

You will primarily serve a diverse patient population ranging from children to the elderly, each presenting unique challenges and conditions. Your role will involve working closely with individuals recovering from surgeries, injuries, neurological disorders, and chronic pain conditions. The focus on home care enables us to deliver tailored support in familiar settings, enhancing rehabilitation efforts and fostering stronger patient-provider relationships.

Qualifications

To be successful in this role, you will need:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• Active state licensure in Physical Therapy.
• Basic Life Support (BLS) certification.
• Strong written and verbal communication skills to engage effectively with patients and their families.
• Proficiency in using electronic medical record (EMR) systems for documentation.

Preferred Experience

While not mandatory, candidates with at least 2 years of clinical experience in home care or outpatient therapy will be highly regarded. A background in orthopedic physical therapy and experience with diverse patient demographics will also be beneficial.

Compensation and Benefits

Mindful Physical Therapy offers a competitive salary ranging from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
